The forms listed here will help you alert us to items of extraordinary value and to act on loss or damge, if any should occur.

Declaration Form

We want to make sure that your prized possessions receive the best possible care during your move. To help us make sure that things go well (and to satisfy an Interstate Commerce Commission requirement) we ask that you declare

• items with a value of $100 per pound or more
• any single item or matching set of items with a value of $2,000 or more
• firearms tendered for transportation (stating make, model, and serial number)

For your convenience, we have provided a Declaration form that you can use to list these items.

Claims Form
